Survey - Create (specific topic)

To create a custom survey:

1. In the Settings menu, select Surveys.
2. Click on the Add a survey button.
3. Select the Custom type.
4. Click on the Create survey button.
5. Fill the form.
6. Enter the questions (in the Questions tab).
